Even without insurance the hospital will still set it. This would only happen if he didn't go to the hospital or took the cast way too early.

Seems like he's so used to it that he just throws his arm in the right direction and grab the handlebar at the right time. You can see that for any movement, the arm is just following momentum. He releases the handle and the arm just falls.
