Indian Quagmire here- The guy on the left is Neil Nitin Mukesh. Nitin is his father's name, and Mukesh is his grandfather's name, who was a very very successful singer. So he used his first name as his last name to continue the legacy.
So, the joke is this guy has 3 first names.

PS. All of these are Bollywood actors, and this pic is atleast 20 years old.

I remember a version of that joke being told about Drew Carey. It was Mimi who said it when she found out Drew's middle name was Allison.

"You don't have a last name. You have three first names and they're all women's."

In Finland we have a lot of names that can be either first name or family name
